Do your eyes burn, sting, or water by the afternoon?
Your eyes feel fine at breakfast and scratchy by three. Screens make it worse. Drops help for a little while, then you are back where you started. Some days your eyes water so much that dry is the last word you would use, yet watering is one of the most common signs of dry eye.
You have plenty of company. A few common medicines dry eyes out, and so do the years after menopause and the hours we all spend on a screen. None of it means you have to live with it, and we would be glad to take a look.
What happens at a dry eye visit?
Most of our patients start with simple care at home: artificial tears, a heated eye mask, lid scrubs, and supplements. Then we ask you to come back for a dry eye evaluation, a visit that looks closer at why your eyes are drying out.
Most dry eye starts in the small oil glands along the edge of the eyelids. When those glands clog or shrink, the oily layer of your tears thins out and your tears dry up too fast. What we find at the evaluation tells us whether you need more than the basics, and which treatment fits.
The dry eye evaluation can include:
A short questionnaire about your symptoms and when they bother you
An InflammaDry test, which checks the surface of your eye for signs of inflammation
Photos of the oil glands inside your eyelids, taken with infrared meibography
Vital dye, colored drops that show us dry or damaged spots on the front of your eye
How do we treat dry eye?
By matching the treatment to what the evaluation finds. Dry eye usually has more than one cause, so many patients use two or three treatments together. We do all of them here in Titusville, so nobody is driving to Orlando for a light therapy session.
The treatments we offer beyond the basics:
Prescription medication, such as drops that calm inflammation on the eye
Punctal plugs: tiny collagen plugs in the tear drains of the lower lids, so more of your own tears stay on the eye
Meibomian gland expression: gentle pressure on the eyelids that helps the oil flow out of the glands more smoothly
OCuSOFT thermal treatment: steady heat and pressure on the eyelids in the office to help the oil glands flow, usually followed by gland expression
LidScrubPro lid scrub: we put on a treatment chosen for your eyelids, then clean the lid edges with a device that works like an electric toothbrush
OUST swabstix: a strong tea tree oil treatment for the lashes, lids, and brows, left on for a few minutes and then wiped off, on its own or before a LidScrubPro scrub
Zocular gel: a lid and lash cleanser made from okra to help calm inflammation, on its own or with a LidScrubPro scrub
Low-level light therapy (LLLT): red light reaches deeper to help the oil glands work, and blue light helps clear the buildup that bacteria and tiny demodex mites leave on the lashes
OptiLight IPL (intense pulsed light): quick flashes of light on the skin around the eyes that reduce the small blood vessels of rosacea, which can calm inflammation and help the oil glands work
Opti-Lift: a treatment without surgery for sagging eyelids that strengthens the muscles around the eyes for a fuller blink, at our Downtown office only
Amniotic membranes: for a cornea that has been slow to heal, we numb the eye and place a thin layer of amniotic tissue on it, held by a contact lens we remove a few days later

Who treats dry eye at Aker Eye, and where?
All four of our doctors, at both offices. Dr. Greg Aker and Dr. Amanda Aker see dry eye at Downtown, 338 S Washington Ave, on the second floor with an elevator, and Dr. Sarah Koehnemann and Dr. Jennifer Bradley see it at Playalinda, 1114 S Washington Ave, on one floor. The dry eye evaluation, the gland imaging, the low-level light therapy, and the OptiLight IPL are at both offices. The one exception is Opti-Lift: that unit cannot move between offices, so an Opti-Lift visit is booked Downtown, whichever doctor is yours.
Does insurance cover dry eye treatment?
Usually the evaluation, yes: dry eye is a medical condition rather than a glasses prescription, so medical insurance tends to cover the visit. Some in-office treatments, such as light therapy, are often not covered and are paid for by the patient. We check your benefits before your visit and tell you what your plan covers and what it does not, so the cost is never news at the desk. What causes dry eyes?
Most dry eye comes from the oil glands in the eyelids clogging or shrinking, so tears dry up too fast. Age, hormone changes, screen time, wind and air conditioning, contact lens wear, some medicines, rosacea, and buildup on the lashes from bacteria and demodex mites all play a part. The cause decides the treatment, which is why we look at the glands before anything else.
What is a dry eye evaluation?
It is a visit that looks for why your eyes are drying out. It can include a questionnaire about your symptoms, an InflammaDry test for signs of inflammation on the eye, photos of the oil glands in your eyelids, and colored vital dye drops that show dry spots on the eye. Most patients try artificial tears, a heated mask, lid scrubs, and supplements first, then come back for the evaluation.
Are eye drops for dry eyes enough?
Sometimes. Artificial tears and lubricating drops ease the symptoms, and for mild dry eye that may be all you need. When the oil glands are clogged, drops alone do not reach the cause, and we look at treatments that get the glands working again. Show us what you have been putting in your eyes and we will tell you whether it is the right kind.
Can dry eye be cured permanently?
Dry eye is usually a long-term condition that we manage rather than make go away for good. It often has more than one cause, so the plan often has more than one treatment. Once we treat the cause instead of the symptom, many patients get lasting relief and need less over time. We tell you plainly what to expect from your plan before you start it.
Is dry eye treatment covered by insurance or Medicare?
The evaluation is usually covered by medical insurance, including Medicare, because dry eye is a medical diagnosis. In-office light therapies are often not covered and are paid for by the patient. We check your benefits before your visit and tell you the cost of anything your plan does not cover.
What is IPL treatment for dry eyes?
IPL stands for intense pulsed light. With OptiLight, we send quick flashes of light over the skin around the eyes to reduce the small blood vessels that rosacea leaves there. That can calm inflammation and help the oil glands work better. After we look at your glands, we tell you whether it is a good fit for you.
