Who you are and how to reach you, your insurance, your eye and health history, and the medicines you take. Keep your card and your medicines list beside you and the whole packet is done in one sitting. Press send and it lands at our front desk.

Your insurance card, your current glasses or contact lens boxes, and a list of your medicines. Set them by the door the night before and you are ready. That is all we need to bill your plan, read your old prescription, and give the doctor the full picture.

About ten minutes. The forms open in your browser, and you can do them on a phone, a tablet, or a computer. Have your insurance card and your list of medicines next to you and it goes faster. Once you send them, they come straight to us, and there is nothing to print or bring.
No. Everything is online, and once you press send it reaches us. If you would rather not do the forms online, come in a few minutes early and we will help you fill them in at the front desk. Either way, the doctor sees you on schedule.
Nothing bad happens. Tell us at the desk and we will help you through them before the doctor sees you. Give yourself a few extra minutes, and bring your insurance card and your medicines list so we are not guessing at either.
Whichever is closer, unless you need a specific doctor. Dr. Greg Aker and Dr. Amanda Aker are at Downtown, 338 S Washington Ave, on the second floor with an elevator. Dr. Koehnemann and Dr. Bradley are at Playalinda, 1114 S Washington Ave, all on one floor. Children usually see Dr. Bradley at Playalinda, and dry eye treatment is at Downtown.
